Admission and Enrollment


Admission Requirements:
    •    Adults 18 years or older
    •    Have a mental illness and/or substance abuse issues
    •    Have the ability to participate  in outpatient clinic services

We will assess your symptoms and needs. You will receive recommendations and referrals
that will be the most helpful to you.  We will inform you of your rights and responsibilities for
mental health and/or substance abuse services.

Please bring:
    •    Picture Identification - Texas ID Card or Texas Driver's License
    •    Social Security card
    •    Proof of Residence (if any)
    •    Proof of Income (if any)

Payment Assistance:
If you are unable to pay for services, come to our clinic to enroll in NorthSTAR.

*LifeNet now accepts private pay, sliding insurance, and sliding scale fees are also available.

How to Enroll:
Come to the LifeNet office.  Intake takes the first ten people to sign up.
Monday through Friday, 8:00 a.m. to 4:00 p.m.
